Report of an accident due to the occurrence of a serious illness or performed surgical intervention

1. to submit a request for insurance compensation (fills in the claim form), once the health condition is allowing
2. to enclose with the request all the necessary documents listed in the list of required documents on the claim form.
3. the accident can only be claimed by the Insured himself.
